Advanced DSRIP project analytics that enable PPSs and partner providers to target high-risk populations and gaps in care highlight the latest enhancement of the SpectraMedix DSRIP Solution
East Windsor, NJ — July 15, 2016 — The SpectraMedix DSRIP Solution’s project analytics capabilities are enabling PPSs and their partner providers to monitor DSRIP project status at a macro level and identify key populations, risks, and trends in near real-time. Understanding performance on a timely basis allows project teams and leadership to make quick adjustments as needed to improve performance and maximize financial incentives.
Projects can be managed using a variety of analyses including identification of patients for engagement, patient risk profiling, utilization of project registries, root cause analysis and rapid cycle improvement, and data / financial model implementation.
At the population level, project analytics can be used to identify target populations for intervention, monitoring and reporting. “Hot spotting” visualizations can identify areas of chronic disease where intervention can significantly improve project performance. Tracking and monitoring of engaged patients, as well as patients overdue for follow up helps to ensure proper care is administered at the right time.
Analytics at the patient level can be used to identify “super utilizers” and low utilizers by examining claims and other data to understand the types of services being used. Implementation of algorithms can help calculate patient risk scores, identify risk factors and store risk data in the data warehouse to facilitate future tracking and monitoring.
